Region­al Com­mu­ni­ty Health Needs Assessment

We need your voice as we determine the top health needs in Onslow County. Our goal is to hear from as many Onslow County residents as possible! We ask that you complete the survey before June 30, 2021.

About the Survey The Onslow County Health Department and Onslow Memorial Hospital are partnering with other health departments and hospitals across the region to conduct Eastern North Carolina’s first ever Regional Community Health Needs Assessment (CHNA). Every few years, residents have an opportunity to take a survey that asks them to identify the top health challenges they face as well as the availability of health resources in the community and what people need to live their healthiest. A regional approach allows individual communities to leverage off of more robust resources, expertise and leadership to improve the overall health of their community members.

Next Steps We estimate that it will take about 20 minutes to complete this 60 question survey.
